Please complete the appropriate intake needed for the service(s) you will be receiving.

If you do not have access to a printer, please be sure to

arrive 20 -30 minutes prior to the start of your

Chiropractic or Acupuncture service.

For Acupuncture you will receive a link to follow to complete your health history and other needed forms via email.  Please be on the lookout for that after you book your appointment.  We will have an ipad in office to complete them as well.  Be sure to allow at least 30 minutes prior to your appointment start time to complete these forms.

Please arrive 15 minutes prior to your massage service

to complete the paperwork in the office, or feel free to complete at home and email it back to us.
